Membranous glomerulopathy with extracapillary proliferation
A case report of overlap with PR3-ANCA-positive vasculitis
Abstract
Introduction: membranous nephropathy (MN) is the leading cause of nephrotic syndrome in adults.
Case presentation: we describe a case of a rare MN variant with extracapillary proliferation, diffuse alveolar hemorrhage (DAH,) and anti-proteinase 3 (anti-PR3) positivity. There were renal histological changes that were atypical for MN (semilunar lesions) and clinically associated with DAH with anti-PR3 positivity.
Conclusion: although rare, cases of DAH with kidney involvement that are ANCA or anti-MBG positive may have renal histological findings of extracapillary proliferative MN.
