Gastrointestinal manifestations of COVID-19 in a Colombian population
A cross-sectional study
Abstract
Introduction: Approximately 20% of patients with COVID-19 have gastrointestinal compromise. This can include sickness, vomit, abdominal pain, diarrhea, and abnormalities in the hepatic function tests. In Colombia, there are not studies of the characteristics of the gastrointestinal compromise in patients with COVID-19.
Objectives: Establish the prevalence of gastrointestinal and liver compromise in patients with COVID-19 treated in two hospitals of Bogotá, Colombia. Establish the association of the gastrointestinal compromise in patients with COVID-19 with long hospital stay, severity of disease and mortality.
Materials and methods: Cross sectional study made in two hospitals of one sub-network of Health Service in Bogotá, Colombia, between February 2020 and March 2021.
Results: We included 1176 patients with COVID-19 confirmed with respiratory RT-PCR. The gastrointestinal manifestations were present in 50% (CI95% 47-52%), the most prevalent manifestation was diarrhea in the 18,4%, odynophagia in the 17,6%, anorexia 14,7% and abdominal pain in 8,8%. There was association of diarrhea and long hospital stay (OR 1.93 CI95% 1.19-3.13), gastrointestinal bleeding with mortality (OR 3.13, CI95% 1.1-9.1), among others. The liver function tests were abnormal in 46% of the patients (CI95% 43-49%), the abnormalities were worst in patients with severe COVID-19 or mortality.
Conclusions: The prevalence of gastrointestinal manifestation in patients with COVID-19 was 50%. The diarrhea was associated with long hospital stay and the gastrointestinal bleeding was associated with respiratory failure and mortality. The abnormalities of liver function tests were present in the 46% of the patients and the transaminases elevations were the most frequent abnormality. The transaminases elevation in the first day of hospitalization was associated with mortality
